I have been dreaming of a pacifier vending thing for the crib. If a baby drops one, they can simply retrieve another one…..now how to make the kid do it themselves at 3 months old?!?!?
I once asked my husband if he could fashion some kind of high plexiglas shield around the crib, so that when our daughter threw her pacifier, it would just bounce right back. He looked at me like I was insane. I thought it was a *fantastic* idea.

You need a “Dropper Stopper!” They’re awesome and very bungee cord like (but safer).

Have you seen the binkie beanie? I just heard about it and think it looks awesome. *IF* we have another someday….

Have you tried links? We used a few of these to keep our kid’s paci attached to the stroller and carseat (he never threw it out of the crib). : http://www.amazon.com/Bright-Starts-Lots-of-Links/dp/B001ABZGU2/ref=sr_1_1?ie=UTF8&qid=1326941871&sr=8-1
I make and sell pacifier clips in my etsy shop: http://www.etsy.com/shop/CalistasBowtique?section_id=8161871
They have been a lifesaver for my daughter!

Or how to keep the dang thing in their mouths!
EXACTLY what I was coming on here to say. If it stayed in their mouth, then no other intervention would be necessary! 😉
Actually, I forgot about this – my girlfriend makes these great bib/pacifier things. http://www.etsy.com/listing/48648144/purple-orange-green-pacifier-bib
